I am not taking new patients at this time. For current patients, I schedule appointments during the week at traditional hours. Pease arrive 10 minutes before your appointment. If a conflict arises please call and cancel your appointment so that another person can use that time.
If you change insurance plans please bring your insurance card and any other information that may be helpful – such as phone numbers for your benefits representative at work, the plan’s case manager for claims, the claims processor for your employer group, or other important people to contact.
Our billing staff will submit claims to your insurance company for all services you receive, and you will be billed for the remaining balance. For a number of reasons, insurance may not provide full coverage for services rendered. Co-pays are due at the time of service.
Our fee schedules will be provided upon request. We do not accept Medicaid or Badger Care. I can certainly see you if you are covered under those plans, but you will be billed at regular charges and will be responsible for the entire amount of the bill. All of your records will be held in strict confidence. I do not use outside transcription services. If you need forms filled out please try to bring them in well in advance to allow me time to complete them accurately.
Payment options include cash or credit card. Personal checks are accepted only under certain situations that have been discussed in advance.
